Requires homeless services providers to submit annual reports to the California Health and Human Services Agency beginning January 1, 2018, containing data on ambulance rides, hospital stays, emergency room visits, arrests, incarceration length, and number of persons served from the previous calendar year.

Mandates data be submitted in an open format that is retrievable, downloadable, indexable, electronically searchable, platform independent, machine readable, and available to the public free of charge.

Requires reported data to be published on the California Health and Human Services Open Data Portal and defines "homeless services provider" by reference to Section 103577 of the Health and Safety Code.

Applies only to homeless services providers that have access to the required data and requires data accuracy to the extent practicable.

Provides that if the Commission on State Mandates determines the bill imposes state-mandated costs, reimbursement shall be made pursuant to Government Code Part 7.
