Requires telephone corporations transitioning to IP-enabled services to complete a customer education and outreach program before withdrawing legacy circuit-switched telephone services, explaining the transition benefits and projected timeframes.

Prohibits withdrawal of voice grade single-line telephone service before January 1, 2020, and requires telephone corporations to provide affected customers with prior notice including information about alternative services available.

Requires the Public Utilities Commission to confirm that alternative services meet specified requirements including voice grade access to the public switched telephone network, real-time two-way voice communications, and 911 emergency access within 120 days.

Allows customers to request commission review within 60 days of receiving withdrawal notice if they believe alternative service is unavailable at their location, and requires the commission to order continued service for up to 12 months if no alternative is available.

Requires the commission to order continued voice service indefinitely if no compliant alternative service becomes available during the initial 12-month extension period.
