Creates the Community-based Health Improvement and Innovation Fund in the State Treasury to support community-based prevention of chronic diseases including asthma, type II diabetes, cardiovascular disease, cancer, dental disease, and obesity, with funds available upon legislative appropriation.

Allocates at least 80 percent of annual appropriations to eligible applicants: at least 47 percent to local health jurisdictions through formula-based three-year funding cycles with a minimum $250,000 base award per jurisdiction, and at least 33 percent for competitive grants to local, regional, and statewide nonprofit organizations.

Reserves up to 20 percent of annual appropriations for the State Department of Public Health for mandatory activities including statewide media campaigns (9 percent), program evaluation (5 percent), and program implementation (6 percent).

Establishes a 13-member Community-based Health Improvement and Innovation Fund Advisory Committee to advise on policy development, evaluation, and recommend strategies for chronic disease prevention, with required triennial submissions of comprehensive recommendations to the Legislature.

Prioritizes investments in health equity priority populations using policy, systems, and environmental change approaches rather than clinical services, with emphasis on communities in the third and fourth quartiles of the California Health Disadvantage Index.
