Requires the California Superintendent of Public Instruction to convene an advisory committee including state board representatives, school districts, parent groups, and advocacy organizations to develop model survey questions aligned with state priorities.

Mandates the Superintendent recommend model surveys to the State Board of Education by July 1, 2017, for consideration and adoption.

Requires adopted model surveys be made available to local educational agencies and the California Collaborative for Educational Excellence to assist with meeting local survey requirements on school safety, connectedness, and parental involvement.

Expresses legislative intent to provide funding to support local educational agencies in implementing the surveys.
