Establishes the Housing for a Healthy California Program requiring HCD to create competitive grant guidelines by October 1, 2017, and award grants annually starting April 1, 2018, subject to legislative appropriation.

Grants must fund long-term rental assistance (up to 5 years), interim housing (up to 9 months), and operating reserves for supportive housing serving homeless Medi-Cal beneficiaries.

Eligible applicants (counties or cities with counties) must identify funding sources for supportive services, commit funding for rental assistance, and agree to collect and report participant data to HCD.

Eligible participants must be homeless, Medi-Cal beneficiaries eligible for Supplemental Security Income, enrolled in supportive services programs, and likely to improve health with supportive housing.

HCD shall report program outcomes and health care cost changes to legislative committees by October 1, 2019, and is exempt from administrative regulations and standard procurement requirements.
