Requires disclosure of medical, dental, and mental health information between state and county correctional and medical facilities when an inmate is transferred to ensure continuity of care, with electronic transmission when possible.

Amends Civil Code Section 56.10 to authorize information sharing between county correctional facilities, county medical facilities, state correctional facilities, and state hospitals for inmate transfers without requiring patient authorization.

Clarifies that incarcerated persons retain all privacy rights legally applicable to inmates, subject to medically necessary sharing of personal medical and mental health information for continuity of care.

Requires all health information disclosures to comply with the Confidentiality of Medical Information Act, state and federal HIPAA regulations, and other applicable privacy and security laws.

Imposes a state-mandated local program by requiring county agencies to disclose inmate health information when jurisdiction transfers, triggering state reimbursement obligations if costs are determined by the Commission on State Mandates.
