Establishes the Aging and Disability Resource Connection (ADRC) program to be administered by the California Department of Aging in collaboration with the Department of Rehabilitation and State Department of Health Care Services.

ADRC programs shall provide enhanced information and referral services, options counseling on long-term services and supports (LTSS) programs, short-term service coordination, and transition services from hospitals and skilled nursing facilities.

Area agencies on aging and independent living centers designated as core local partners in developing ADRC programs within their geographic areas.

Program becomes operative only upon appropriation of funds by the Legislature.
