Requires each county to convene a working group with stakeholders to discuss and assess resources needed for creating or supporting juvenile transition centers that provide housing and services to youth released from juvenile correctional facilities.

County juvenile transition centers must provide temporary transitional housing and services including education, work training, health and mental health services, family and social services, and legal services.

Counties must coordinate with other county, local, state programs, and nonprofit organizations to provide and maximize services.

Participation in transition centers is voluntary and cannot be made a condition of release, parole, or probation.

Counties are only required to implement the program to the extent that funds are available from the state or other sources.
