Requires counties to use either a prepopulated renewal form or a blank semiannual report form as the certificate of eligibility for CalWORKs and CalFresh programs.

Department must ensure the semiannual report form contains all information required by federal law and regulation for Temporary Assistance for Needy Families (TANF) and Supplemental Nutrition Assistance Program (SNAP) eligibility.

Each adult family member must provide information under penalty of perjury to complete the certificate, or mark any changes if a prepopulated form is used.

Counties must mail certificates no later than the end of the month prior to when due, with a postage-paid return envelope, and must attempt personal contact before terminating benefits for incomplete certificates.

For recipients receiving CalFresh benefits, the certificate must be completed according to timeframes required by federal and state law for the CalFresh program.
