Requires the State Department of Social Services to include questions in initial application forms and semiannual report forms (effective January 1, 2019) that allow applicants and recipients to disclose disabilities, accommodation needs, and domestic violence experiences.

Mandates that any statewide automated welfare system include a notification visible to caseworkers upon opening an applicant's or recipient's file indicating whether they have disclosed a disability, need for accommodation, or domestic violence experience that may affect eligibility for CalWORKs exemptions.

Requires the State Department of Social Services to collaborate with county welfare departments in developing the caseworker notification system to ensure compliance with the Americans with Disabilities Act.
