Expands the right to add written addenda to patient records to all patients regardless of age, previously limited to adult patients and minors authorized to consent to medical treatment.

Limits each addendum to 250 words per alleged incomplete or incorrect item and requires the patient to clearly indicate in writing that the addendum be made part of the record.

Requires health care providers to attach addenda to patient records and include them in any disclosure of the disputed portion to third parties.

Protects health care providers from liability for defamatory or unlawful language contained in patient addenda that is included in records in accordance with the law.

Establishes that violations are punishable under existing criminal provisions in Health and Safety Code Section 123110(i) and Section 123120.
