Authorizes counties to establish a homeless adult and family multidisciplinary personnel team to facilitate expedited identification, assessment, and linkage of homeless individuals to housing and supportive services.

Permits team members from agencies including mental health, law enforcement, medical services, social services, veterans services, domestic violence organizations, schools, and housing providers to share confidential information for coordinating services.

Requires each county to develop protocols governing information sharing that must be posted on the county website within 30 days and submitted to the State Department of Social Services.

Mandates all team members receiving information be subject to the same privacy and confidentiality obligations as the person disclosing the information, with violations subject to existing civil and criminal penalties.

Specifies that team discussions and information exchanges are confidential and testimony about them is inadmissible in criminal, civil, or juvenile court proceedings.
