Requires health insurers and health care service plans (excluding Medi-Cal managed care plans) to cover at least one version each of buprenorphine, methadone, naltrexone, buprenorphine-naloxone combination, and naloxone approved by the FDA for opioid use disorder treatment.

Prohibits prior authorization, annual or lifetime dollar limits, step therapy, fail-first policies, and treatment duration limitations for at least one version of each covered medication-assisted treatment drug.

Applies to drugs prescribed to enrollees as of January 1, 2019, or at the time of enrollment for new enrollees, and does not restrict coverage based on prior treatment success or failure.

Includes sunset provision repealing the requirements on January 1, 2024.

Specifies no state reimbursement is required for local agencies or school districts under Article XIII B of the California Constitution.
