State Registrar must issue up to 3 certified copies per year of live birth records without fee to homeless persons or homeless children/youth, with discretion to provide additional copies.

Homeless person or authorized representative can request certified birth records by submitting an affidavit signed by both the applicant and a qualified homeless services provider verifying housing status.

Qualified homeless services providers include nonprofits receiving federal/state/local funding, attorneys, school liaisons, human services providers funded by California, and law enforcement liaisons to homeless populations.

State Department of Public Health must develop the verification affidavit and can implement this section through director instructions without formal rulemaking.
