Requires health care service plans and health insurance policies issued after January 1, 2019, to limit cost sharing for noncontracting air ambulance providers to the same amount as in-network providers.

Establishes that enrollees and insureds cannot owe noncontracting air ambulance providers more than the in-network cost-sharing amount, and such amounts count toward deductibles and out-of-pocket limits.

Directs the State Department of Health Care Services to set Medi-Cal reimbursement rates for air ambulance services equal to a percentage of rural Medicare rates, effective July 1, 2019, subject to federal approval.

Modifies the Emergency Medical Air Transportation and Children's Coverage Fund to use remaining money for offsetting the state portion of Medi-Cal reimbursement rates rather than augmenting payments after the new rate is implemented.

Creates a state-mandated local program by making willful violations of health care service plan requirements a crime, with penalties terminating January 1, 2020.
