Authorizes the probation department of the committing county to participate in a juvenile ward's reentry case conference with the Division of Juvenile Justice, either in person or by video conferencing.

Requires the Division of Juvenile Justice to provide the probation department and committing court with the ward's most recent written review not less than 60 days prior to a scheduled discharge consideration hearing.

Allows the probation department to submit a written reentry supervision plan to the Division of Juvenile Justice not less than 30 days prior to the discharge consideration hearing, which the Board of Juvenile Hearings must consider when determining whether to release the ward.

Requires the committing court to conduct a reentry disposition hearing no more than four days but no less than one day after the Division of Juvenile Justice transfers the ward to county probation custody.

Makes technical and conforming changes to existing juvenile discharge procedures under the Welfare and Institutions Code.
