Extends the initial Medi-Cal eligibility period from 6 months to 12 months for families who lose CalWORKs assistance due to increased employment income or loss of earned income disregards, effective January 1, 2019.

Eliminates the optional additional 6-month extension period and related provisions that allowed the department to offer continued coverage beyond the initial extension under federal law.

Requires the State Department of Health Care Services to implement the 12-month option available under federal law (42 U.S.C. Section 1396r-6(a)(5)).

Removes language requiring the department to impose premiums during an additional extension period, as that period will no longer exist under the new structure.

Imposes state-mandated local program obligations on counties, which are responsible for making Medi-Cal eligibility determinations; provides for reimbursement through existing statutory procedures if the Commission on State Mandates determines costs are mandated by the state.
