Requires the Office of Systems Integration to ensure CalHEERS transfers an individual's health care benefits application to their county of residence if they are potentially eligible for CalFresh benefits and opt into applying.

Mandates counties treat applications transferred from CalHEERS as CalFresh applications and process them according to CalFresh program provisions.

Requires the Department of Health Care Services to issue guidance to county human services agencies on how to consider CalHEERS applications as CalFresh applications.

Establishes that if the Commission on State Mandates determines the bill creates state-mandated costs, reimbursement to local agencies shall be made pursuant to Government Code Part 7.
