Amends Section 4701 of the Probate Code to revise the statutory advance health care directive form used in California.

Revises Part 3 of the form relating to organ, tissue, and parts donation by allowing individuals to authorize their agent to consent to temporary medical procedures necessary to evaluate and maintain organs for donation purposes.

Allows individuals to specify the purposes for their donation (transplant, therapy, research, or education) and add restrictions or preferences regarding organ donation.

Clarifies that leaving the donation section blank does not constitute a refusal to donate, and permits authorized individuals to make donation decisions if no agent is named.

Approved by the Governor on September 6, 2018, with no fiscal impact or local program effects.
