Ensures In-Home Supportive Services (IHSS) remain available through Medi-Cal managed care health plans in eight specified counties (Alameda, Los Angeles, Orange, Riverside, San Bernardino, San Diego, San Mateo, and Santa Clara) regardless of whether the Coordinated Care Initiative generates net General Fund savings.

Establishes the California In-Home Supportive Services Authority as the entity to negotiate wages, benefits, and employment terms with employee organizations for IHSS individual providers, with an advisory committee including at least 50% current or former IHSS users.

Implements a County IHSS Maintenance of Effort (MOE) requirement starting with a 2011-12 base year and annually adjusting by 3.5% inflation, with counties paying this amount instead of their traditional cost-sharing percentage.

Allows managed care health plans to contract with certified qualified agencies to provide IHSS services, with contract providers receiving wages and benefits equal to those negotiated for individual providers.

Appropriates $650,000,000 from the General Fund to the Department of Health Care Services to fund IHSS provision under these provisions.
