Expands grounds for resident transfers to include conditions that are "detrimental to" health, safety, or well-being, not just those that "endanger" it.

Requires continuing care facilities to use specified assessment tools during the transfer evaluation process and share completed assessments with residents or their responsible persons.

Mandates providers submit medical reports, documentation of current mental and physical function, prognosis, and expected duration of conditions for disputed transfers, with copies provided to residents.

Requires the State Department of Social Services Continuing Care Contracts Branch to describe steps taken and factors considered in transfer decisions, and to specify whether the transfer is appropriate and necessary.
