Establishes the Dignity at Home and Fall Prevention Program requiring the California Department of Aging to provide grants to area agencies on aging for injury prevention services for older adults and persons with disabilities.

Grants must fund injury prevention information, education, home environmental assessments, and services including grab bars, handrails, medical alert devices, and home modifications for eligible individuals.

Eligibility limited to individuals with household income not exceeding 80 percent of area median income who have fallen, are at risk of falling, or at risk for institutionalization and cannot otherwise afford these services.

Requires grant applicants to submit plans detailing projected clients to be served, geographic service locations, array of services offered, and ability to collaborate with local health and injury prevention agencies.

Grant funding capped at $150,000 per eligible area agency on aging and subject to legislative appropriation through the Budget Act or other statute.
