Mandates the report include data on children referred for case management services, specifying those who received home visits, environmental investigations, and family education or nutrition services.

Requires the report to identify sources of lead exposure for children with lead poisoning and indicate whether those sources have been removed, remediated, or abated.

Directs the department to post the report prominently on its Internet Web site and provide the collected data to the Healthy Communities Data and Indicators Project.

Requires compliance with all applicable state and federal privacy and security laws when using or disclosing data under this section.
