Removes exemption for licensed optometrists holding a branch office license from obtaining a statement of licensure to practice at branch office locations.

Eliminates branch office licensing requirements and instead prohibits any optometrist or group of optometrists from operating more than 11 offices total.

Requires licensed optometrists to notify the State Board of Optometry of their practice addresses before engaging in optometry and obtain statements of licensure for all locations except their principal place of practice.

Maintains exemption for temporary practice of optometry at locations other than the principal office for up to 5 calendar days per 30-day period and 36 days per calendar year.

Specifies that violations of office limitations result in suspension of the optometrist's license, which may be restored only upon compliance and payment of required fees.
