Dissolves the elected board of directors of the West Contra Costa Healthcare District effective January 1, 2019, and removes all remaining members from office.

Requires the Contra Costa County Board of Supervisors to either serve as the district board themselves or appoint a five-member district board consisting of county and local residents.

If the Board of Supervisors appoints a district board, it must include at least one county supervisor and at least one city council member from a city within district boundaries.

Allows the Board of Supervisors to determine whether appointed district board members serve at their pleasure or for staggered four-year terms with removal for cause.

Addresses the district's financial struggles since the mid-1990s, including bankruptcies in 2006 and 2016, with the governance change intended to reduce administrative costs and increase operational efficiencies.
