Requires state or local initiative, referendum, or recall petitions paid for by a committee to include a disclosure statement identifying the committee name, top contributors (those with cumulative contributions of $50,000 or more), and the date top contributors were calculated.

Mandates disclosure statements be updated within 14 days of any change in top contributor identities, with paid circulators required to use updated petitions within 21 days.

Requires committees employing paid circulators to submit disclosure statements and updates to the Secretary of State for posting on the Secretary of State's website.

Specifies disclosure format requirements including white background, black border box, 10-point Arial type, and prohibition on signature invalidation due to absent or inaccurate disclosure information.
