Mandates the master plan consider a universal assessment tool and process to streamline access to long-term services and supports through coordinated eligibility determination across fragmented programs.

Specifies the universal assessment tool should evaluate individual needs and determine initial eligibility for programs and services in the long-term services and supports delivery network.

Requires the Master Plan for Aging Stakeholder Advisory Committee to seek input from consumers, providers, and advocates when considering the universal tool and process.

Addresses California's aging population projected to increase by four million people over 65 by 2030, while noting current system fragmentation creates barriers to accessing necessary services.
