Requires skilled nursing facility licensees to notify residents and their representatives in writing at least 90 days before a sale, transfer of operation, management change, assignment, lease, or other ownership change, including specified information about the prospective licensee, owners, shareholders, and expected date of change.

Imposes a civil penalty of $100 per day on licensees who fail to comply with the notification requirements.

Prohibits prospective licensees from discharging employees (except nursing home administrator and director of nursing) without cause during a 60-day transition period following ownership change.

Requires that employees retain their wages, benefits, and other terms and conditions of employment without reduction during the 60-day transition period.

Exempts skilled nursing facilities operated as distinct parts of acute care hospitals and applies only to license applications submitted after specified dates (July 1, 2020 for notification requirements; January 1, 2020 for employment protections).
