Requires University of California, California State University, and California Community Colleges campuses to update former student records with legal name or gender changes upon receipt of government-issued documentation (driver's license, birth certificate, passport, social security card, or court order).

Mandates institutions reissue documents including transcripts and diplomas with updated legal names or genders when requested by former students.

Requires institutions to allow graduating students beginning with the 2022–23 class to request diplomas list their chosen name without requiring legal documentation to prove a name or gender change.

Designates the bill as a state-mandated local program, with reimbursement procedures to be determined by the Commission on State Mandates per existing government code provisions.
