Requires the California Department of Aging to develop and publish an informational tool by July 1, 2021 to help individuals and caregivers communicate with health care providers after an Alzheimer's disease diagnosis.

Tool must be based on the 2003 publication "Partnering With Your Doctor: A Guide for Persons with Memory Problems and Their Care Partners" jointly developed by the Alzheimer's Association and the former State Department of Health Services.

Department must update information from the 2003 publication and ensure the tool addresses the needs of California's increasingly diverse aging population.

Department shall evaluate the tool's effectiveness at least once every three years, contingent on funding availability.

Implementation must use existing department resources with no new appropriation required.
