Renames the California Firefighters' Memorial Fund and California Peace Officer Memorial Foundation Fund to the "Voluntary Tax Contribution Fund" versions and extends the sunset date from January 1, 2021 to January 1, 2028

Allows taxpayers to continue designating voluntary tax contributions on personal income tax returns for both memorial funds through the extended deadline

Requires the California Fire Foundation and California Peace Officers Memorial Foundation, Inc. to annually post distribution information on their websites detailing how funds are used

Maintains a $250,000 minimum annual contribution threshold for each fund to remain on the tax return form, with automatic adjustment for inflation beginning in 2005

Permits contributions to be used for memorial maintenance and repair, ceremonies honoring fallen firefighters and peace officers, survivor assistance programs, and related activities
