Lowers the age threshold for "elder" from 65 to 60 years old and for "dependent adult" from 18-64 to 18-59 years old, and specifically identifies those with traumatic brain injuries or cognitive impairments as dependent adults.

Expands the Home Safe Program to include services supporting housing transitions and exempts grantees from matching funds on any grant money exceeding the 2019 Budget Act base level.

Requires counties to establish provisions for homeless prevention and longer-term housing assistance through the Home Safe Program and mandates referral to appropriate agencies when homeless elders or dependent adults have serious mental illness or substance use disorders.

Establishes the Adult Protective Services FAST/Forensic Center Grant Program to award grants to counties for creating Financial Abuse Specialist Teams and forensic centers composed of multidisciplinary professionals to review complex elder and dependent adult abuse cases.

Requires the State Department of Social Services to convene a workgroup and submit recommendations by November 1, 2021, for creating a statewide adult protective services case management or data warehouse system.
