Requires the Department of Housing and Community Development to create and administer a rental registry online portal by January 1, 2022, to collect information from landlords who own or operate 5 or more rental units and accept COVID-19-related rental assistance from federal or state funds.

Mandates landlords submit rental registry forms for each property by April 1, 2022, and annually thereafter by March 31, providing information on property address, ownership, unit occupancy, rent amounts, rent increases, tenant terminations, and affordable housing restrictions.

Prohibits landlords who receive COVID-19 rental assistance from issuing rent increase notices, notices to terminate tenancy, or initiating eviction actions unless they have submitted a completed rental registry form.

Allows non-landlords including tenants, local governments, courts, and nonprofit organizations to submit information to the portal, subject to departmental verification before public release.

Establishes that landlords completing the registry form receive an Annual Statement of Registration certificate and that no state reimbursement is required due to the state-mandated local program provisions.
