Extends the $4 penalty imposed on Vehicle Code and local ordinance violations (excluding parking offenses) from July 1, 2020 expiration to July 1, 2021, with collection continuing until December 31, 2022.

Directs courts to transfer collected penalty revenues to the Emergency Medical Air Transportation and Children's Coverage Fund for Medi-Cal reimbursement of air ambulance services and children's health care coverage.

Allocates fund distributions as follows: administrative costs first, then 20% of remaining funds to offset state Medi-Cal reimbursement rates and 80% to augment emergency medical air transportation provider payments.

Provides that no state reimbursement is required for this local mandate because local agencies have authority to levy sufficient service charges or fees under Government Code Section 17556.

Declares the act an urgency statute taking effect immediately to maintain funding for Medi-Cal air ambulance services after the prior penalty assessment expired.
