Requires each county to form a multidisciplinary team collaborating with the Department of Housing and Community Development, housing authorities, educational entities, and child-focused organizations to serve nonminor dependents in extended foster care.

Mandates counties submit annual reports to the State Department of Social Services documenting educational status, housing status, lifelong connections, sibling connections, long-term transition plans, and homelessness or housing insecurity for each nonminor dependent under their jurisdiction.

Requires the State Department of Social Services to submit an annual report to the Legislature on nonminor dependent status and foster care service improvements, post the report on its website, and present findings at child welfare conferences and forums.

Directs the department to establish a working group by January 1, 2022, in consultation with the California Youth Connection and other stakeholders, to ensure existing foster care tools function properly and develop training curricula for child welfare professionals working with nonminor dependents.

Specifies the state shall reimburse local agencies only to the extent that state funding is provided for costs exceeding the 2011 Realignment Legislation baseline.
