Expands the definition of "health care provider" to include speech-language pathologists, audiologists, physician assistants, and nurse practitioners under medical records access laws.

Requires health care providers to provide free copies of medical records to employees of nonprofit legal services entities representing patients when records are needed for public benefit appeals or certain immigration petitions.

Expands the list of qualifying public benefit programs to include the Cash Assistance Program for Aged, Blind, and Disabled Legal Immigrants and government-funded housing subsidy or tenant-based housing assistance programs.

Prohibits health care providers from charging fees for filling out forms or providing medical information responsive to forms supporting claims or appeals for public benefit program eligibility.

Allows health care providers to honor requests to disclose patient records or complete public benefit forms containing the written or electronic signature of the patient or personal representative.
