Requires law enforcement agencies to complete a supplemental hate crime or hate incident report form for each suspected hate crime or hate incident

Report form must indicate the type of bias motivation and other identifying information to assist in hate crime prosecution

Defines "hate incident" as a noncriminal act committed because of actual or perceived characteristics including disability, gender, nationality, race or ethnicity, religion, sexual orientation, or association with persons having these characteristics

Hate incident reporting is limited to informational, crime prevention, law enforcement planning, trend analysis, and evidentiary purposes and does not authorize penalties for hate incidents alone

Establishes state reimbursement procedures for local agencies and school districts if the Commission on State Mandates determines the bill creates mandated costs
