Establishes the California Department of Community Living within the California Health and Human Services Agency to consolidate leadership on services for older adults, people with disabilities, and caregivers.

Requires the department to establish a statewide No Wrong Door system to provide older adults and people with disabilities with accurate information, timely referrals, assessment services, options counseling, and community transition services.

Mandates the department to provide leadership and streamline access to home and community-based services including independent living centers, nutritional programs, caregiver support, assistive technology, and waiver programs.

Requires the department to provide block grants to area agencies on aging for services including Alzheimer's day care, the Brown Bag program, and Senior Companion/Foster Grandparent programs, with local discretion in fund allocation.

Directs the department to maintain current funding levels for independent living centers as of January 1, 2021, and increase access to the Aging and Disability Resource Connections program through funding and technical assistance.
