Establishes a 3-year pilot program in Alameda County, beginning no earlier than January 1, 2021, to provide medically supportive food as a covered Medi-Cal benefit for beneficiaries with chronic health conditions including diabetes, heart disease, hypertension, kidney disease, prediabetes, and obesity.

Defines medically supportive food to include healthy food boxes, groceries, meals, food vouchers, and renewable food prescriptions using evidence-based practices that demonstrate prevention, reduction, or reversal of specified chronic diseases.

Authorizes the Department of Health Care Services to establish utilization controls on the number and frequency of services and enter into contracts with public, nonprofit, and community-based organizations, prioritizing those providing California-grown produce.

Requires the department to evaluate the pilot program upon completion and submit findings and recommendations to the Legislature by January 1, 2024, or within 12 months after program conclusion, whichever is sooner.

Implements the program through provider bulletins without regulatory action, conditioned on federal law compliance, availability of federal funding, and federal approval, with the program repealing on January 1, 2026.
