Prohibits licensed health care professionals from charging fees for completing certification forms required for In-Home Supportive Services (IHSS) and Medi-Cal personal care option services.

Requires applicants or recipients of IHSS to obtain certification from a licensed health care professional declaring they cannot independently perform activities of daily living and are at risk of out-of-home placement.

Directs the Department of Social Services to develop a standard certification form for use across all counties in consultation with the State Department of Health Care Services and stakeholders.

Allows counties to accept alternative documentation such as hospital discharge plans, nursing facility assessments, or individual program plans to meet certification requirements.

Requires counties to provide recipients with written notice of certification requirements in languages spoken by substantial numbers of people served by the IHSS program.
