Establishes the Medical Foster Home Pilot Program until January 1, 2023, allowing United States Department of Veterans Affairs (USDVA) facilities to create medical foster homes exempt from state residential care facility licensing and regulation.

Medical foster homes must meet federal requirements under Title 38 of the Code of Federal Regulations (Sections 17.73 and 17.74) and receive approval from the USDVA's Director of Home and Community-Based Care in Geriatrics and Extended Care Services.

Requires USDVA facilities establishing medical foster homes to submit to California State Auditor jurisdiction and provide data, information, and case files upon request for program evaluation.

Mandates that medical foster home caregivers and other non-veteran residents over 18 years of age must be registered independent home care aides, with USDVA facilities verifying registration during monthly visits.

Directs the California State Auditor to conduct an audit of the pilot program no sooner than January 1, 2021, evaluating USDVA facility compliance and making recommendations to the Legislature regarding program continuation.
