Expands homeless multidisciplinary personnel teams in Los Angeles, Orange, Riverside, San Bernardino, San Diego, Santa Clara, and Ventura counties to identify and prevent homelessness for individuals at risk of homelessness, in addition to serving currently homeless individuals.

Defines "individual at risk of homelessness" as an indigent person receiving public benefits who is exiting or recently exited detention/treatment facilities or aging out of child welfare/juvenile justice systems and who has significant health, mental health, or substance use issues.

Requires team members to notify individuals at risk of homelessness that their confidential information may be shared for housing and supportive services and to attempt obtaining consent before sharing information.

Permits team members to share confidential information about individuals at risk of homelessness for identification purposes (unless individual opts out) or for coordinating services for up to 30 days (unless individual opts out earlier).

Authorizes team membership to include case managers or case coordinators responsible for referral, linkage, or coordination of care and services; repeals the at-risk homelessness provisions on January 1, 2025.
