Applicants must disclose information about operators, managers, owners, and beneficial owners with 10% or more interest, including their prior service in similar facilities within the past 10 years

Department may deny applications or revoke licenses if applicants knowingly withhold material information, make false statements, or fail to disclose required administrative disciplinary actions

Establishes civil penalty of $1,000 for material violations of licensing application requirements, in addition to other remedies under the chapter
