Requires county probation agencies and social workers to include a postdischarge plan in case plans prior to discharge from short-term residential therapeutic programs (STRTP), covering at least 6 months of services and supports for the minor and placement family.

The postdischarge plan must be developed in consultation with the child and family team and clinical staff, and shall consider the minor's CANS assessment, clinical indicators, mental health needs, wraparound services, educational activities, peer supports, extracurricular activities, respite care, and specialized care increments.

Prohibits extension of STRTP placement if the sole reason for extension is development of the postdischarge plan.

Applies same postdischarge plan requirements to nonminor dependents receiving AFDC-FC benefits up to age 21.

Designates this bill as imposing a state-mandated local program, with no state reimbursement required under specified conditions related to the 2011 Realignment Legislation.
