Establishes temporary California Community Transitions program to help Medi-Cal beneficiaries transition from inpatient facilities to community settings, regardless of length of stay in facility.

Allows transitioning individuals who have resided in an inpatient facility for fewer than 90 days to receive services consistent with the federal Money Follows the Person Rebalancing Demonstration.

Prohibits enrollment of new beneficiaries under the program starting January 1, 2023, and requires cessation of all services under the program starting January 1, 2024.

Authorizes the Department of Health Care Services to implement the program through letters and provider bulletins without formal regulatory action.

Automatically repeals the program on January 1, 2025, and takes effect immediately as an urgency statute to address COVID-19 transmission risks in institutional settings.
