Establishes the Farm to Community Food Hub Program administered by the Office of Farm to Fork to increase local and sustainable food purchasing by schools and institutions while supporting the farming economy.

Authorizes farm to community food hubs operated by nonprofits, school districts, cooperatives, businesses, or Certified B Corporations to aggregate and distribute products from local farms prioritizing sustainable practices and fair labor standards to public institutions and food banks.

Requires a 10-member advisory committee with representatives from three California regions, food supply chain businesses, civil society organizations, the Sustainable Agriculture and Research Education Program, and disadvantaged or beginning farmers.

Implements the program in two phases: Phase 1 awards planning grants for proposals to create food hubs; Phase 2 awards development grants to at least 3 selected proposals for 5 years of capital and operating expenses.

Requires annual legislative reports for at least 5 years after development grant awards on financial viability, farms served, and institutions supplied; program sunsets January 1, 2028 unless extended by legislation.
