Establishes Medi-Cal eligibility for incarcerated individuals in state prisons or county jails for 30 days before their release date if they otherwise meet eligibility requirements.

Requires the Department of Health Care Services to submit annual reports to the Legislature by November 30 detailing the number of inmates served, program costs, and effectiveness metrics including emergency room visits, hospitalizations, and overdose rates after release.

Authorizes the department to implement the program through provider bulletins and guidance documents before January 1, 2026, when formal regulations must be promulgated.

Conditions implementation on obtaining federal approvals, including state plan amendments or waivers, and requires federal matching funds to be obtained.

Establishes that if the Commission on State Mandates determines the bill creates state-mandated local costs, reimbursement shall be made pursuant to existing statutory provisions.
