Requires the Department of Motor Vehicles to enter into a memorandum of understanding with Donate Life California to mutually agree upon language for organ and tissue donor enrollment on driver's license and identification card applications.

Allows applicants to designate a voluntary contribution of $2 or more (previously limited to $2) when applying for an original or renewal driver's license or identification card to support organ and tissue donation programs.

Requires the DMV to electronically transmit weekly to Donate Life California the names, addresses, birth years, and license numbers of all applicants who enroll in the organ donation program.

Authorizes persons under 18 years of age to register as donors, with the legal guardian making the final decision regarding donation.

Creates the Donate Life California Trust Subaccount in the Motor Vehicle Account to receive voluntary contributions, with funds used to support organ donation programs after deducting the DMV's administrative costs.
