Establishes the Office of the Healthy Brain Initiative within the State Department of Public Health to oversee all department activities related to Alzheimer's disease and implement action items from the federal Healthy Brain Initiative.

Requires the office to operate a program in at least 10 local health jurisdictions (including up to 2 rural and at least 1 coastal jurisdiction) and award one-time grants to develop local initiatives addressing brain health, workforce competence, and program evaluation.

Mandates the office to evaluate community solutions implemented by participating jurisdictions and submit a consolidated report to the Legislature by December 31, 2025, with updated reports every three years thereafter.

Expands the Alzheimer's Disease and Related Disorders Advisory Committee from 20 to at least 21 but not more than 25 members, adding representatives for first responders, local health jurisdictions, primary care physicians, and a California Commission on Aging commissioner with relevant expertise.

Adds 2 legislative ex officio members (one Senator and one Assembly member) to the advisory committee and removes the one-year term requirement for members with Alzheimer's disease or related disorders diagnosis.
