Requires health care service plans, health insurance policies, and self-insured employee welfare benefit plans issued, amended, or renewed on or after January 1, 2023, to cover screening mammography, medically necessary diagnostic or supplemental breast examinations, and related tests upon referral by participating healthcare providers.

Supplemental breast examinations and tests must be covered consistent with nationally recognized, evidence-based guidelines.

Prohibits cost-sharing (deductibles, coinsurance, copayments) for covered screening mammography, diagnostic, and supplemental breast examinations, except for high deductible health plans before the deductible is satisfied for the year.

Defines diagnostic breast examinations as evaluations of abnormalities or examinations based on personal/family medical history or genetic mutations, and supplemental breast examinations as screenings when no abnormality is suspected but risk factors exist.

Exempts specialized health insurance, Medicare supplement insurance, CHAMPUS supplement insurance, TRICARE supplement insurance, and hospital indemnity or specified disease insurance from these requirements.
